The information you provide during identity proofing is pulled directly from your credit report. This means you need an exact match for each field.

Tip 2
If there is a period in a prescriber’s name, it may cause identity proofing to fail. Example: M. Anna Smith.
Please update the NPI registry to remove the period from your name to pass identity proofing. You may put the period back once identity proofing is complete.
Tip 3
It is recommended to register two tokens to your account to e-prescribe controlled substances.
If you lose access to a token and do not have a back up token registered, you may lose access to e-prescribe controlled substances until you redo identity proofing and EPCS registration.
Tip 4
Depending on your token's manufacturer (e.g., Symantec, Onespan) and whether it's an application or a physical device, you will format the serial number differently when you associate token(s) to your account.
- VIP Access app: SYMC###########, SYMD###########, or VSST########### (no spaces)
- Symantec hard token: AVT########### or FT########### (no spaces or dashes)
- OneSpan hard token: ########### (no spaces or dashes)
Tip 5
You may attempt to pass identity proofing 3 times in a 24-hour period.
- After 3 attempts, your account will be locked for the full 24-hour period.
- This is a security measure and cannot be unlocked.
Tip 6
Fraud Alert does not need to be removed from your account before going through identity proofing. To complete IDP with a fraud alert in place, complete optional fields as well as required fields.
Tip 7
If you do not pass identity proofing, re-enter the required fields plus the optional field for credit card number (Visa or Mastercard).
Only the first 8 digits are required, and it must be a personal credit card, not a business card or a personal debit card.
Tip 8
If you still cannot pass identity proofing, it could mean a few things:
- You've moved within the last 6 years, and the Best on File address isn’t the current address you are living at.
- Your address is misspelled.
- Ensure that while entering your address during identity proofing to leave out any special characters or periods (Ex. 1 E. Main St. Apt. # 204 - enter as 1 E Main St Apt 204).
- Your financials are tied to a PO box rather than your residential address. In this case, you will need to put your address as the PO Box address.
- Your date of birth or Social Security Number may be missing form your credit profile.
Tip 9
You have a non-US addresses (Canada, outside of the US), therefore a US address tied to your financials is required to pass identity proofing.
